To help you prepare for your visit with us, please download the first three new patient intake forms below. Complete them and bring them in along with your insurance card and photo ID card to your first visit. If your insurance required you to have a referral to see us, please have your PCP complete the Consult Request Form prior to your visit with us.

WHAT SERVICES DOES HOPE INTEGRATIVE CARE OFFER?

We provide outpatient detox and long term addiction treatments for alcohol and opioid addiction. Our doctor has extensive experience in pain management with osteopathic manipulation techniques, acupuncture and therapeutic cupping.

WHAT IS BUPRENORPHINE AND VIVITROL? AND, WHICH IS BETTER FOR ME?

Buprenorphine (Bunavail, Suboxone, Zubsolv, Sublocade) is an FDA approved medication which significantly reduces opioid cravings. Its unique pharmacologic properties help patients who struggle with opioid addiction remain fully functional as if they don’t have addiction. Vivitrol is a new drug which can help with alcohol and opioid cravings. It blocks the euphoric feelings from using or drinking. Superiority depends on individual’s addiction history, treatment received.

WHAT ARE PAYMENT SERVICES AND FEES?

You may choose to self-pay or use health insurance. We accept major credit cards for your convenience. If you are experiencing financial difficulties, you may qualify for certain special payment schedules. Co-payments and deductibles are payable at the time of the appointment and are a patient’s responsibility.

WHAT IS YOUR CANCELLATION POLICY?

If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, a no show fee may be applied. Missed or cancelled appointment fees are not covered by health insurance and are a patient’s responsibility.
